Recovery Coach - Rehabilitation Specialist - Case Manager

Job Summary:

Summary of Job:  Provides multiple stage individual and group treatment in the office and community settings to develop trusting relationships with clients so that they may more freely discuss substance and its effects on mental and physical health and daily functioning.  May participate in setting treatment goals and plans with clients to reduce or abstain from substance use and to learn replacement behaviors.

Essential Duties and Responsibilities:

Facilitates client’s use of family, social and community resources;  acting as an advocate with external resources and agencies and self-help programs.

Provide individual, group and family counseling covering psycho-educational, addictions, daily living skills and activity therapy.

Develop, implement and monitor clinically and fiscally responsible individualized treatment/discharge plans.

Participate in treatment team meetings for the purpose of summarizing, reviewing and developing client’s treatment goals, progress and service utilization.

Maintain necessary communications and reporting with referral sources and other involved parties as approved by the client; provides testimony in court in regards to client’s behavior and condition when requested.

Job Qualifications:  Bachelors Degree from a four-year college or university is required, preferably in social work, psychology or related field.  Experience working or volunteering in the behavioral health, criminal justice or social services fields is preferred, but not required.
